Exploration of Risk Factors for Bleeding Complications during Extracorporeal Membrane Pulmonary Oxygenation Support in Adult Peripheral Catheterization

Abstract：Objective:To explore the risk factors for bleeding complications during the process of extracorporeal membrane oxygenation support in adult peripheral catheterization.Methods:From November 2022 to November 2024,72 adult patients with peripheral catheterization and extracorporeal membrane oxygenation support were selected as the subjects.Based on whether bleeding complications occurred during the patient support process,they were divided into a bleeding group and a non bleeding group.Various clinical data and examination results of the patients were summarized,and multiple factor regression analysis was conducted on the risk factors of bleeding complications.Results:Among the 72 patients,24 developed bleeding complications,with a rate of 33.33%.There were statistically significant differences in mechanical ventilation time,acute physiology and chronic health evaluationⅡ(APACHEⅡ)score,sepsis-related organ failure(SOFA)score,fibrinogen(FIB),and D-dimer(D-D)between the bleeding group and the non bleeding group(P＜0.001).Logistic regression analysis showed that prolonged mechanical ventilation time,high APACHE Ⅱscore,high SOFA score,low FIB,and high D-D were independent risk factors for bleeding complications during adult peripheral catheterization and extracorporeal membrane oxygenation support,with statistically significant differences(P＜0.05).Conclusion:There is a high probability of bleeding complications during the extracorporeal membrane oxygenation support process in adults with peripheral catheterization.Mechanical ventilation time and coagulation function are the main risk factors,and early prevention of these factors can reduce the risk of bleeding complications.
