Study on the Effects of Percutaneous Endoscopic Nucleus Pulposus Removal through Different Approaches on the Rehabilitation Process and Traumatic Stress of Patients with Lumbar Disc Herniation

Abstract:Objective:To explore the effects of percutaneous endoscopic transforaminal discectomy(PTED)through different approaches on the rehabilitation process and traumatic stress in patients with lumbar disc herniation(LDH).Methods:A total of 94 patients with LDH admitted to Ji'an Central People's Hospital from October 2020 to October 2023 were selected as study objects.They were divided into Group A(n=47)and Group B(n=47)based on the differences in treatment plans.All patients were PTED therapy,group A was given the posterior interlaminar apporach,group B was given the transforaminal lumbar approach the road.Comparing rehabilitation process,lumbocrural pain and lumbar function,traumatic stress index levels,the postoperative complications between two groups.Results:The duration of surgery and length of hospital stay in group A were significantly shorter than those in group B,and the intraoperative blood loss was also lower,with statistically significant significance(P<0.001).In low back pain visual analogue scale(VAS)score,leg pain VAS score and Oswestry disability index(ODI)score of the two groups at 6 months after surgery were lower than those before surgery,and group A was lower than group B,the differences were statistically significant(P<0.05).The levels of prostaglandin E2(PGE2),tumor necrosis factor(TNF)-α,and cortisol(Cor)in both groups of patients were higher than those before surgery on day 1 after surgery,but the levels in group A were lower than those in group B,and the differences were statistically significant(P<0.001).There was no significant difference in the incidence of postoperative complications between the two groups(P>0.001).Conclusion:Compared with the transforaminel lumbar approach,the posterior interlaminar approach PTED has better effects,and can promote rehabilitation of the LDH patients,relieve pain,improve the lumbar vertebral function,and can reduce the traumatic stress.
