Comparison of HFNC and NIPPV Ventilation Strategies in Patients with Severe Pneumonia Complicated with Respiratory Failure

Abstract:Objective:To investigate the efficacy of high-flow nasal cannula oxygen therapy(HFNC)and non-invasive positive pressure ventilation(NIPPV)in the treatment of severe pneumonia(SP)complicated with respiratory failure(RF).Methods:A total of 63 patients with SP combined with RF admitted in the First People's Hospital of Zhumadian from March 2023 to May 2024 were selected and divided into control group and observation group according to different treatment methods.Based on routine treatment,the control group(n=33)were treated with NIPPV and the observation group(n=33)were treated with HFNC.It compared the clinical efficacy,incidence of adverse reactions of the two groups,as well as pulmonary function,blood gas,sputum viscosity and serum inflammatory factors level before and after treatment.Results:The total effective rate of the observation group was higher than that of the control group(P<0.05).After treatment,the ratio levels of forced expiratory volume in one second(FEV1),peak expiratory flow(PEF),FEV1/forced vital capacity(FVC)in observation group were higher than those in control group(P<0.05).After treatment,partial pressure of carbon dioxide(PaCO2)level in observation group was lower than that in control group,arterial partial oxygen pressure(PaO2)and peripheral oxygen saturation(SpO2)levels were higher than that in control group(P<0.05).After treatment,the proportion of grade Ⅱ and Ⅲ in observation group was higher than that in control group,and the proportion of grade Ⅳ was lower than that in control group(P<0.05).After treatment,the levels of high mobility group box-1 protein(HMGB-1),interleukin(IL)-6,soluble myeloid cell trigger receptor(sTREM)-1,tumor necrosis factor(TNF)-α in the observation group were decreased more than those in the control group(P<0.05).There was significant difference in the incidence of adverse reactions between the two groups(P<0.05).Conclusion:HFNC combined with conventional methods is effective in the treatment of SP combined with RF,which can not only improve lung function,reduce sputum viscosity,but also inhibit inflammation in vivo,with fewer adverse reactions,and has high clinical value.
