The Prognostic Value of Extravascular Lung Water Index in Acute Respiratory Distress Syndrome with Extracorporeal Membrane Oxygenation

Abstract:Objective:To evaluate the prognostic value of the extravascular lung water index(ELWI)in patients with adult respiratory distress syndrome(ARDS)treated with extracorporeal membrane oxygenation(ECMO).Methods:A retrospective analysis of 68 patients with ARDS admitted to the sample hospital from January 2010 to December 2022 was conducted to collect the detailed clinical data,including ECMO parameters,respiratory settings,and pulse indicator continuous cardiac output measurements.Differences between the suivival group(n=36)and the death group(n=32)were compared.The predictive value of ELWI at different time points for suivival to discharge was assessed using receiver operating characteristic(ROC)curves.Results:ELWI was higher in the death group at 24h,48h,72h,and 1 week post-ECMO initiation,and at 72h and 1 week,the differences were statistically significant(P<0.05).The ROC curve showed that 72h ELWI was the best predictor of survival,with an area under the curve of 0.823(95%CI 0.730-0.917),a cutoff value of 13.6 mL/kg,sensitivity of 81%,and specificity of 68%.Conclusion:ELWI at 72 hours post-ECMO holds high predictive value for ARDS patients.
