The Impact of Immediate Postoperative Gemcitabine Infusion Chemotherapy on Recurrence and Survival Following TURBT for NMIBC

Abstract:Objective:To evaluate the impact of immediate postoperative gemcitabine bladder instillation chemotherapy on the recurrence rate and survival time of patients with non-muscle invasive bladder cancer(NMIBC)following transurethral resection of bladder tumor(TURBT).Methods:A total of 126 NMIBC patients treated at the First Affiliated Hospital of Henan University of Science and Technology between July 2020 and July 2023 were selected for the study.They were divided into two groups based on different treatment protocols:the conventional group(60 patients)and the immediate group(66 patients).The conventional group received gemcitabine bladder instillation chemotherapy one week after TURBT,while the immediate group received the instillation immediately after the surgery.The clinical efficacy,recurrence rates at 6-month and 1-year follow-ups,and 1-year survival rate were compared between the two groups.Kaplan-Meier survival curves were used to analyze the 1-year survival time post-surgery.Results:The total clinical efficacy in the immediate group was significantly higher than in the conventional group(P<0.05).At both 6-month and 1-year follow-ups,the recurrence rate in the immediate group was significantly lower than that in the conventional group(P<0.05).Survival analysis showed that the 1-year cumulative survival rate and survival time were both significantly better in the immediate group compared to the conventional group(P<0.05).Conclusion:Immediate postoperative gemcitabine bladder instillation chemotherapy can significantly reduce the recurrence rate and extend the survival time of NMIBC patients.It is more effective than the conventional chemotherapy regimen starting one week post-surgery.Immediate instillation chemotherapy provides an effective treatment option for improving the postoperative prognosis of NMIBC.
