Comparative Analysis of Non-inflatable Oral Vestibular Approach and Under-Chin Approach in the Treatment of Papillary Thyroid Carcinoma

Abstract：Objective:To compare the efficacy of cavity thyroidectomy in the treatment of papillary thyroid carcinoma between non-inflatable oral vestibular approach and under-chin approach.Methods:A total of 85 patients with papillary thyroid admitted to Huizhou Central People's Hospital from August 2022 to December 2023 were selected as the study subjects,and were divided into the non-inflatable oral vestibular approach group(n=40)and the under-chin approach group(n=45)in accordance with the difference of surgical access methods.The intraoperative related indexes,postoperative recovery related indexes,complication occurrence,health economics indexes and incision satisfaction were compared between these two groups.Results:Surgery was successfully completed in both groups,with no intraoperative openings;there was no statistically significant difference between these two groups in terms of operative time,dietary recovery time,postoperative drainage and time required for postoperative extubation,pain,occurrence of complications,and hygiene and economic indexes(P＞0.05);the intraoperative bleeding in the non-inflatable oral vestibular approach group was larger than that of the under-chin approach group,and the operative time was lower than that of the under-chin approach group,the difference was statistically significant(P＜0.001);the numerical satisfaction scale(NSS)score of patients in the non-inflatable oral vestibular approach was higher than that of the under-chin approach group,and the difference was statistically significant(P＜0.05).Conclusion:In the surgical treatment of papillary thyroid cancer,compared with the under-chin approach,the non-inflatable oral vestibular approach has a shorter operation time,a clearer field of vision,better protection of the recurrent laryngeal nerve,and is safer,with a more hidden incision,and a higher degree of patient satisfaction.
