Meta-Analysis of Factors Affecting Extubation Failure in Mechanically Ventilated Patients

Abstract：Objective:To systematically analyze the factors influencing extubation failure in mechanically ventilated patients and provide evidence-based guidance for reducing extubation failure rates in this population.Methods:A systematic review of observational studies on factors influencing extubation failure in mechanically ventilated patients was conducted using computerized searches of CNKI,Wanfang,VIP,CBM,Web of Science,PubMed,Cochrane Library,and Embase.The search period spanned from database inception to February 2024.Two researchers independently performed literature screening,quality assessment,and data extraction according to standardized protocols.Meta-analysis was performed using Stata 15.0 statistical software.Results:A total of 48 studies were included,with a combined sample size of 22 391 participants,and 32 factors were extracted.Meta-analysis results indicate that factors influencing extubation failure in mechanically ventilated patients include duration of mechanical ventilation,age,Acute Physiology and Chronic Health Evaluation II score(APACHE II score),diabetes,multiple organ dysfunction syndrome(MODS),presence of respiratory disease,serum albumin(ALB),respiratory shallow breathing index(RSBI),partial pressure of carbon dioxide(PaCO2),respiratory rate at extubation,smoking status,lung ultrasound B-line score(LUBS),hemoglobin(HB),diaphragmatic dysfunction,Glasgow Coma Scale(GCS)score,white blood cell count(WBC)at extubation,heart rate,B-type natriuretic peptide(BNP),blood urea nitrogen,chronic obstructive pulmonary disease(COPD),renal disease,C-reactive protein(CRP),and heart failure(P＜0.05).Conclusion:Numerous factors contribute to extubation failure in mechanically ventilated patients.Healthcare providers should develop a scientifically detailed extubation plan,closely monitor relevant factors,and reduce the risk of extubation failure for patients.
