Abstract:Objective:To analyze the association between peripheral blood miR-210 expression levels and disease progression in neonatal pulmonary arterial hypertension(PAH).Methods:A retrospective analysis of the general and clinical data of 91 pediatric patients with congenital heart disease(CHD)admitted to Jiaozuo Maternal and Child Health Hospital from June 2022 to March 2024 and were divided into a PAH group(31 cases)and a non-PAH group(60 cases). The correlation between peripheral blood miR-210 and mean pulmonary artery pressure(mPAP),pulmonary capillary wedge pressure(PCWP),and pulmonary vascular resistance(PVR)was examined.Risk factors contributing to secondary PAH in pediatric patients with CHD were analyzed and summarized. The predictive value of peripheral blood miR-210 for secondary PAH in CHD was validated. Results:Apgar scores and mean PCWP at birth were lower in the PAH group than in the non-PAH group. The percentage of cesarean sections, cases with intrauterine infection, amniotic fluid contamination, patent ductus arteriosus(PDA),respiratory distress syndrome(RDS),meconium aspiration syndrome(MAS),respiratory acidosis, and neonatal asphyxia were higher in the PAH group than in the non-PAH group.Additionally, peripheral blood miR-210 expression levels,mPAP,and PVR were significantly higher in the PAH group, with statistically significant differences(P<0.05).Peripheral blood miR-210 negatively correlates with PCWP and positively correlates with mPAP and PVR(P<0.05).Concurrent PDA,RDS,MAS,elevated miR-210 expression,mPAP,PCWP,and PVR constitute risk factors for secondary PAH in pediatric CHD patients.Peripheral blood miR-210 demonstrates high sensitivity and specificity in predicting secondary PAH in pediatric CHD patients. Conclusion:Dynamic monitoring of miR-210 expression levels enables early prediction of the risk of developing PAH in pediatric patients.
