Clinical Study of Driving Pressure-guided PEEP Titration Combined with Ambroxol on the Efficacy and Impact on Pulmonary Compliance and Inflammatory Mediators in Patients with Acute Respiratory Distress Syndrome

Abstract:Objective:To explore the clinical efficacy of driving pressure(ΔP)-guided positive end-expiratory pressure(PEEP)titration combined with ambroxol in the treatment of acute respiratory distress syndrome(ARDS)and its effects on pulmonary compliance and inflammatory mediators.Methods:Selecting 60 ARDS patients admitted to Guangdong Suixi People's Hospital from April 2024 to April 2025,they were randomly assigned to the control group(ΔP-guided PEEP)and the observation group(ΔP-guided PEEP combined with ambroxol).The differences in oxygenation,pulmonary compliance,and inflammatory mediators between the two groups were assessed.Results:The observation group showed a significantly higher partial pressure of oxygen(PaO2)/fraction of inspiration(FiO2)ratio,improved pulmonary dynamic compliance,and significantly lower levels of inflammatory markers compared to the control group,with statistically significant differences(P<0.05).Conclusion:ΔP-guided PEEP titration combined with ambroxol significantly improves oxygenation,enhances pulmonary compliance,and effectively reduces inflammation in ARDS patients,showing promising clinical application prospects.
