Effect of Hepatic Lobular Hepatectomy on Perioperative Indices and Near-term Outcome in Patients with Hepatic Bile Duct Stones Combined with Bile Duct Stenosis Using Hepatic Lobular Hepatectomy

Abstract：Objective:To investigate the effect of hepatic lateral lobular hepatectomy on perioperative indices and recent outcomes in patients with hepatic bile duct stones combined with bile duct stenosis.Methods:83 patients with hepatic bile duct stones combined with bile duct stenosis admitted to Nanyang City Center Hospital from June 2023 to June 2024 were retrospectively selected as the study subjects,and the grouping method was based on different surgical modalities,with the control group adopting choledochotomy and lithotripsy together with T-tube drainage(n=41),and the study group adopting hepatic lateral lobular hepatectomy(n=42).Perioperative indicators,liver function,stress indicators and complications were compared between the two groups.Results:The duration of surgery,postoperative exhaustion and hospitalization of patients in the study group was shorter than that of the control group,and intraoperative bleeding was lower than that of the control group,with statistically significant differences(P＜0.05).There was no statistically significant difference between the liver function indexes and stress indexes of patients in the two groups before surgery(P＞0.05).Postoperative liver function and stress indicators of patients in both groups were lower than before surgery,with statistically significant differences(P＜0.05),and all liver function indicators and stress indicators of patients in the study group were lower than those of the control group,and the difference was statistically significant(P＜0.05).The incidence of postoperative complications was lower in the study group(9.52%)than in the control group(26.83%),and the difference was statistically significant(P＜0.05).Conclusion:Lateral lobular hepatectomy in hepatobiliary stones combined with bile duct stenosis is effective in improving patients'perioperative indexes and liver function,reducing postoperative stress reactions,and lowering the risk of complications.
