The Effects of Restrictive Fluid Resuscitation on Inflammatory Response,Oxygen-Carrying Capacity and Coagulation Function in Patients with Severe Multiple Trauma Combined with Hypovolemic Shock

Abstract:Objective:To explore the effects of restricted fluid resuscitation protocol on coagulation function,inflammatory response degree and blood oxygen-carrying capacity in patients with severe multiple injury combined with hypovolemic shock in emergency department.Methods:A total of 93 emergency patients with severe multiple injuries accompanied by hypovolemic shock admitted to the Emergency Department of Zhengzhou Central Hospital Affiliated to Zhengzhou University from November 2021 to March 2024 were selected as the study objects.They were divided into the control group(46 cases)and the observation group(47 cases).Both of these two groups were given damage control surgery,the control group were given active fluid resuscitation program,and the observation group were given restricted fluid resuscitation program.The blood coagulation function,blood oxygen carrying capacity,inflammatory reaction degree,complication rate and mortality were compared between these two groups.Results:After resuscitation,prothrombin time(PT)and activatedpartial-thromboplastin-time(APTT)levels in observation group were lower than the control group.The arterial partial pressure of oxygen(PaO2)level,pH value,NF-Kb,miR-155 level and peptide in the observation group were higher than the control group at 90 min and 120 min after resuscitation,the arterial partial pressure of carbon doxide(PaO2),TNF-α level,complication incidence and mortality of the observation group were lower than the control group,and the difference was statistically significant(P<0.05).Conclusion:The use of restricted fluid resuscitation in the treatment of severe multiple injuries with hypovolemic shock could effectively improve the oxygen-carrying capacity of the patients,reduce the level of inflammatory indicators,improve coagulation function,and reduce the occurrence of complications and mortality.
