Changes in NT-proBNP,cTnI,and sST2 Expression in Elderly Patients with Acute Heart Failure and Their Clinical Significance

Abstract:Objective:To investigate the expression changes and clinical significance of N-terminal pro-brain natriuretic peptide(NT-proBNP),cardiac troponin I(cTnI),and soluble ST-2(sST2)in elderly patients with acute heart failure(AHF).Methods:A retrospective analysis was conducted on 100 elderly patients with acute heart failure(AHF)admitted to Donghai County People's Hospital in Jiangsu Province from May 2022 to June 2023.Based on disease severity,57 non-severe patients(NYHA Class Ⅰ-Ⅱ)were assigned to the control group,while 43 severe patients(NYHA Class Ⅲ-Ⅳ)were assigned to the observation group.NT-proBNP,cTnI,and sST2 levels were compared between the two patient groups,along with the incidence of major adverse cardiovascular events(MACE).Receiver operating characteristic(ROC)curves were plotted to determine the predictive value of NT-proBNP,cTnI,and sST2 for AHF,and factors influencing the occurrence of cardiac MACE were analyzed.Results:Patients in the observation group exhibited significantly higher levels of NT-proBNP,sST2,and cTnI compared to the control group,with statistically significant differences(F=2.153,3.522,3.855;P<0.05).The combined detection of serum NT-proBNP,sST2,and cTnI demonstrated higher area under curve(AUC),sensitivity,and specificity compared to individual markers(P<0.05).Patients in the event group exhibited significantly higher NT-proBNP,sST2,and cTnI levels before and after treatment compared to the non-event group,with statistically significant differences(t=2.249,2.496,4.060,2.345,10.251,11.293;P<0.05).Elevated NT-proBNP,sST2,and cTnI levels were all significant predictors of cardiac MACE occurrence(P<0.001).Conclusion:The combined detection of NT-proBNP,cTnI,and sST2 serves as a crucial biochemical indicator for the early diagnosis of elderly patients with acute heart failure(AHF),and the changes in the expression of these three markers are closely associated with the occurrence of major adverse cardiac events(MACE)in patients.
