Analysis on the Efficacy and Safety of 1.5J×40Hz Holmium Laser Enucleation for Benign Prostatic Hyperplasia

Abstract:Objective:To explore the clinical feasibility,efficacy,and safety of holmium laser enucleation of the prostate(HoLEP)with the parameter of 1.5 J×40 Hz in the treatment of benign prostatic hyperplasia(BPH)in elderly males.Methods:The clinical data of 120 BPH patients requiring surgical treatment from June 2019 to December 2022 were reviewed and analyzed,and was divided into control group[transurethral resection of the prostate(TURP)group]and observation group[holmium laser enucleation of the prostate(HoLEP)group]by the random number method,with 60 patients in each group.The two groups were compared in terms of surgical duration,perioperative complications,indwelling time of drainage tube,time of bladder irrigation,and average postoperative hospital stay.Additionally,comparisons were made between the two groups regarding the degree of reduction in post-void residual urine volume(PVR),the magnitude of change in International Prostate Symptom Score(IPSS),as well as the improvement in Quality of Life Score(QOL)and maximum urinary flow rate(Qmax)in patients before surgery and 8 weeks after surgery.Results:At 8 weeks postoperatively,there were no statistically significant differences between two groups in terms of the degree of reduction in PVR,as well as the improvement in IPSS and QOL(P>0.05).However,the improvement in Qmax in the observation group was significantly better than that in the control group(TURP group),and the incidence of urinary incontinence in the observation group was lower than that in the control group.Additionally,the surgical duration(minutes),number of days of bladder irrigation,indwelling time of urinary catheter,and length of hospital stay in the observation group were all shorter than those in the TURP control group,and the differences were statistically significant(P<0.05).Conclusion:1.5 J×40 Hz HoLEP is safe and reliable in the treatment of prostatic hyperplasia,and its efficacy is better than TURP.
