Clinical Value of Serum Fibrinogen and D-Dimer Level Measurement for Bacterial Infection Detection in COPD Patients

Abstract:Objective:To evaluate the clinical value of serum fibrinogen(Fib)and D-dimer(D-D)level measurements in detecting bacterial infections in patients with chronic obstructive pulmonary disease(COPD).Methods:A total of 120 patients with COPD infection admitted to the Third Affiliated Hospital of Luohe Medical College from January 2023 to June 2024 were selected as COPD infection group.Another 120 patients with COPD without infection during the same period were selected as the COPD group.Additionally,120 check up participants without COPD were randomly selected from the same period's health examinations to serve as the control group.Blood samples were collected from all three groups of subjects at the hospital,and serum Fib and D-D levels were measured.The differences in serum marker levels among the three groups of patients were compared.COPD patients with infections were divided into bacterial and viral groups based on the type of infection,and the serum Fib and D-D levels of patients in different groups were analyzed to assess their diagnostic value for bacterial infections in COPD patients.Results:Among the three groups of patients,the COPD infection group had the highest serum Fib and D-D levels,followed by the COPD group,which had higher serum coagulation index levels than the control group,with statistically significant differences(P<0.05).In COPD infection patients,the serum Fib and D-D levels in the bacterial infection group were higher than those in the viral infection group,with statistically significant differences(P<0.05).Through receiver operating characteristic(ROC)analysis,the area under the curve(AUC)of Fib combined with D-D for the diagnosis of bacterial infection was 0.931,the sensitivity was 84.6%,and the specificity was 91.5%,all higher than those of a single indicator,and the differences were statistically significant(P<0.05).Conclusion:Serum Fib and D-D levels are significantly elevated in COPD patients with infections,and serum Fib and D-D levels can effectively distinguish bacterial infections.
