The Correlation between Serum PCT,CXCL12 and the Effect of Second-line Chemotherapy in Elderly Patients with Advanced Non Small-cell Lung Cancer

Abstract：Objective:To analyze the correlation between serum procalcitonin(PCT),chemokine(C-X-C motif)ligand 12(CXCL12)and the efficacy of second-line chemotherapy in elderly patients with advanced non small-cell lung cancer(NSCLC).Method:From July 2021 to January 2023,81 patients with advanced NSCLC who failed first-line treatment were selected as the research object,and were divided into uncontrolled group(40 cases)and control group(41 cases)according to the disease control of second-line chemotherapy.The serum PCT,CXCL12 and baseline data of the two groups were compared,and the related factors affecting the effect of second-line chemotherapy were analyzed by logistic regression.Receiver operating characteristic(ROC)curve was drawn to test the predictive value of serum PCT,CXCL12 and their combination on the effect of second-line chemotherapy among elderly patients with advanced NSCLC.Results:After treatment,the serum PCT and CXCL12 levels of all patients decreased,and the difference was statistically significant(t=3.708,4.457;P＜0.05).The serum PCT,CXCL12,carcinoembryonic antigen(CEA),carbohydrate antigen 199(CA199),CA125 and cytokeratin 21-1 fragment(CYFRA21-1)in the uncontrolled group were significantly higher than those in the control group,and the difference was statistically significant(t=2.138,5.026,3.033,3.526,5.944,6.439;P＜0.05).CA199,serum PCT and CXCL12 were the influencing factors of second-line chemotherapy in elderly patients with advanced NSCLC(P＜0.05).The area under the curve(AUC)of serum PCT and CXCL12 in predicting NSCLS patients'effect of second-line chemotherapy were 0.794,0.834 and 0.966,respectively,the combined predictive value of PCT and CXCL12 was better than that of single index(P＜0.05).Conclusion:Serum PCT and CXCL12 levels are related to the effect of second-line chemotherapy in elderly patients with advanced NSCLC,and have certain predictive value.
