Study on the Correlation between Bilirubin Blood Lipid Composite Index and Vulnerable Carotid Plaques in Patients with Acute Cerebral Infarction

Abstract:Objective:To explore the correlation between bilirubin lipid composite index and vulnerable plaques in the carotid artery of patients with acute cerebral infarction.Methods:Selecting 100 patients with acute cerebral infarction who were treated in Nanyang Central Hospital from January 2023 to June 2024 as research objects.The patients were divided into a vulnerable group(n=50)and a stable group(n=50)based on their carotid artery condition.At the same time,50 healthy physical examinees were selected as the control group.Fasting venous blood samples were collected from patients and healthy physical examinees to measure indicators such as total bilirubin(TBIL),direct bilirubin(DBIL),total cholesterol(TC),low-density lipoprotein cholesterol(LDL-C),high-density lipoprotein cholesterol(HDL-C),and triglycerides(TG).Calculating the bilirubin lipid composite index 1[TC/(HDL-C+TBIL)]and bilirubin lipid composite index 2[LDL-C/(HDL-C+TBIL)].Results:There was no statistically significant difference in the history of hypertension,diabetes,smoking and drinking between the patients with acute cerebral infarction and the healthy people(P>0.05).However,there were differences in fasting blood glucose(FBG),TBIL,DBIL,indirect bilirubin(IBIL),TC,TG,HDL-C and LDL-C.There were statistically significiant differences in FBG,TBIL,TC,TG,HDL-C,LDL-C between vulnerable group patients and stable group patients(P<0.05),which could be used as risk factors for carotid vulnerable plaques in patients with acute cerebral infarction,among them,HDL-C was negatively correlated,while the rest were positively correlated;the bilirubin lipid composite index 1 and bilirubin lipid composite index 2 of patients with vulnerable plaques were significantly lower than those without vulnerable plaques(P<0.05),and showed a negative correlation with the formation of vulnerable plaques in the carotid artery(r=-0.702,-0.714;P<0.05).Conclusion:The bilirubin lipid composite index has significant value in identifying vulnerable plaques in the carotid artery of patients with acute cerebral infarction,which provides scientific basis for plaque stability and risk prediction infarction,as well as important guidance for personalized prevention and intervention strategies.
