Application of Quantitative Parameters of Energy Spectrum CT in Predicting Microvascular Invasion in Preopera-tive Hepatocellular Carcinoma

Abstract:Objective:To explore the application value of quantitative parameters of energy spectrum CT(GSI)in preoperative prediction of microvascular invasion(MVI)in preoperative hepatocellular carcinoma(HCC).Methods:A total of 100 HCC patients admitted to the hospital from November 2021 to November 2023 were selected.All patients underwent enhanced energy GSI scans before surgery,including 44 cases of MVI and 56 cases of non MVI.The iodine concentration(IC),standardized iodine concentra-tion(NIC),water concentration(WC),effective atomic number(Eff-Z),and 40 kiloelectronvolts(40 keV)CT values during the arte-rial and venous phases were compared in both groups.Receiver Operating Characteristic(ROC)curve was drawn,and the predic-tive value of the quantitative parameters for the occurrence of MVI in patients was analyzed.Results:In comparision of the non MVI group,the CT values of venous phase volume IC,NIC,Eff-Z and arterial phase IC,NIC,Eff-Z,40 keV in the MVI group were higher(t=4.104,7.978,4.146,9.182,13.046,5.828,5.032;P<0.05).The results of logistic regression analysis showed that arterial phase IC(OR=2.931),arterial phase NIC(OR=1.785)and arterial phase 40 keV CT value(OR=1.976)were independent risk factors for MVI in CRC patients(P<0.05).The ROC curve showed that the areas under the curve for HCC MVI patients were 0.651,0.701,and 0.665 for arterial phase IC,NIC,and 40 keV CT values,respectively.Conclusion:There is significant dif-ference in the quantitative parameters of energy spectrum CT between MVI group patients and non MVI group patients.The upper arterial phase IC,NIC and 40 keV CT values can provide a more reliable basis for the preoperative incidence of HCC microvascu-lar invasion.
