Study on the Effect of Stepwise Nutritional Intervention on Nutritional Status and Prognosis of Pancreatic Cancer

Abstract:Objective:To observe the effect of stepwise nutritional intervention on nutritional status and prognosis of patients with pancreatic cancer.Methods:A prospective study included 85 patients with pancreatic cancer admitted to the First Affiliated Hospital of Henan University from February 2020 to February 2023.Patients were randomly assigned to the conventional group(n=42)and the stepped group(n=43).The routine group was under routine management,and the stepwise group was under combined routine management with stepwise nutritional intervention.Comparing the nutritional status,immune function,occurrence and survival of intestinal and extraintestinal complications between the two groups.Results:After intervention,compared with the conventional group,the patients in the stepwise group had higher body mass index(BMI),triceps skinfold thickness(TSF),and upper arm muscle circumference(AMC),and the differences were statistically significant(t=2.361,2.394,2.812;P<0.05).Compared with the conventional group,the levels of CD3+,CD4+,and CD4+/CD8+in the stepped group were higher,and the differences were statistically significant(t=2.632,2.253,5.382;P<0.05).Compared with the conventional group,the total incidence of enteral and parenteral complications in the stepwise group was lower,and the difference was statistically significant(χ2=6.818,P<0.05).Compared with the conventional group,patients in the stepwise group had higher Karnofsky Performance Status(KPS)score,higher quality of life(QOL)score,and lower Zubrod-ECOG-WHO performance status(ZPS)score.The differences were statistically significant(t=2.203,4.259,2.268;P<0.05).Conclusion:Stepwise nutrition intervention can improve the nutritional status of patients with pancreatic cancer and enhance their immune function,reduce the risk of intestinal and extraintestinal complications,and improve the quality of life of patients.
