Clinical Study on the Treatment of Thyroid Tumors with Radical Resection of Unilateral Thyroid Cancer Using MGUAA

Abstract:Objective:To explore the effect of modified gasless unilateral axillary approach(MGUAA)unilateral radical resection of thyroid carcinoma(RRTC)of thyroid carcinoma in the treatment of patients with thyroid tumor(TT).Methods:The data of 90 patients with TT in the hospital from February 2022 to February 2024 were collected,and they were divided into two groups according to the different operation schemes.45 patients who received unilateral RRTC treatment of MGUAA were included in the MGUAA group,and 45 patients who received traditional unilateral COT treatment were included in the traditional group.The perioperative indicators,visual analogue scale(VAS)scores 1 d,3 d,and 7 d after operation,vancouver scar scale(VSS)scores 1 month,2 months,and 3 months after operation,and the levels of T lymphocyte subsets(CD3+,CD4+,CD4+/CD8+),the incidence of complications were compared.Results:The operation time of the MGUAA group was longer than that of the traditional group,the intraoperative blood loss was less than that of the traditional group,and the hospitalization time was shorter than that of the traditional group,with statistically significant differences(t=13.731,69.371,12.602;P<0.05).The VAS scores of the MGUAA group were lower than those of the traditional group 1 d,3 d and 7 d after operation,with statistically significant differences(t=12.785,13.624,24.936;P<0.05).The VSS scores of the MGUAA group were lower than those of the traditional group 1,2,and 3 months after operation,with statistically significant differences(t=5.415,5.826,6.530;P<0.001).The CD3+,CD4+and CD4+/CD8+of the MGUAA group were higher than those of the traditional group 1 d and 3 d after operation,with statistically significant differences(t=8.391,6.928,5.794,7.851,7.738,4.314;P<0.001).Compared with the incidence of complications in the two groups,the incidence of complications in the MGUAA group was lower than that in the traditional group,with statistically significant difference(χ2=4.444,P<0.05).Conclusion:Compared with the traditional unilateral COT treatment for TT patients,MGUAA unilateral RRTC treatment is more conducive to optimizing perioperative indicators,improving scar aesthetics,reducing postoperative pain,and reducing the risk of complications.It has less impact on the level of T lymphocyte subsets.
