Predictive Value of Systemic Immune-Inflammation Index for Lower Limb Deep Vein Thrombosis after Nephrectomy for Renal Cell Carcinoma

Abstract:Objective:To evaluate the predictive value of systemic immune-inflammation index(SII)for postoperative lower extremity deep venous thrombosis(LEDVT)in patients with renal cell carcinoma(RCC).Methods:A retrospective case-control study was conducted,including 155 patients who underwent radical nephrectomy in the Department of Urology,Tianjin Third Central Hospital from January 2019 and March 2023.Multivariate analysis was used to screen the independent risk factors of postoperative LEDVT.Results:Compared with the non-LEDVT group,the LEDVT group had a higher age and body mass index,higher SII,preoperative Autar score and plasma D-dimer level,lower albumin level,and a later tumor stage.The differences between the groups were statistically significant(P<0.05).Multivariate logistic regression analysis indicated that SII,tumor stage and plasma D-dimer were independent risk factors for LEDVT,and hazard ratios were 5.24(P<0.001),4.26(P<0.001)and 2.97(P=0.018).Receiver operating characteristi curve(ROC)curve analysis showed that SII had the strongest discriminative performance in predicting postoperative LEDVT with an area under the ROC curve(AUC)of 0.764,with the optimal cut-off 623×109/L.Kaplan-Meier survival curves showed that the incidence of LEDVT in the high-SII group was significantly higher than that in the low-SII group(χ2=15.039,P<0.001).Conclusion:Preoperative SII level can be used as an independent risk predictor of LEDVT after radical nephrectomy.
