Risk Factors of Post-polypectomy Bleeding in Patients with Giant Colon Pedicle Polyps After EMR

Abstract:Objective:To explore the risk factors of post-polypectomy bleeding(PPB)in patients with giant colon polyps after endoscopic mucosal resection(EMR).Methods:A total of 64 patients with giant colon pedicle polyps after EMR in the hospital from April 2021 to April 2024 were enrolled,including 32 cases with PPB in PPB group and 32 cases without PPB in non-PPB group.General data,such as age,gender and comorbidities,as well as clinical characteristics such as the number and location of polyps of the two groups were collected.Logistic regression analysis was used to analyze the risk factors of PPB after EMR treatment.Results:There were statistically significant differences in BMI,postoperative feeding time,and history of antithrombotic drug use between the PPB group and the non-PPB group(χ2=5.540,4.016,4.730;P<0.05).The differences in the number of polyps,the location of polyps,intraoperative bleeding,and the pedicle diameter between the PPB group and the non-PPB group were statistically significant(χ2=5.107,4.016,4.267;t=2.037;P<0.05).The results of logistic regression analysis showed that postoperative food intake≤48 hours(OR=2.812),use of antithrombotic drugs(OR=3.842),the number of polyps>5(OR=2.147),and right hemicolonic polyps(OR=2.100)were independent risk factors affecting PPB in patients with large colonic thick-pedicled polyps after EMR treatment(P<0.05).Conclusion:Eating within 48 h after surgery,use of anti-thrombus drugs,number of polyps>5 and polyps of right hemicolon are independent risk factors of PPB in patients with giant colon pedicle polyps after EMR.Clinically,operation by senior physicians or placement of titanium clips in advance can reduce incidence of postoperative PPB.
