Analysis of Lymphocyte Subsets and Inflammatory Mediators in Alveolar Lavage Fluid of Patients with NSCLC and Immune Checkpoint Inhibitor-associated Pneumonia

Abstract:Objective:To analyze the levels of lymphocyte subsets and inflammatory mediators in alveolar lavage fluid of patients with non-small cell lung cancer(NSCLC)complicated with immune checkpoint inhibitor-associated pneumonia(CIP).Methods:The clinical data of 84 patients with NSCLC who were treated with immune checkpoint inhibitors in the hospital were collected from July 2022 to July 2023.The patients were divided into CIP group(n=32)and non-CIP group(n=52)according to whether they were complicated with CIP.The levels of lymphocyte subsets(CD3+,CD4+,CD8+and CD4+/CD8+)and inflammatory mediators[interleukin-6(IL-6),interleukin-8(IL-8)and interleukin-10(IL-10)]in alveolar lavage fluid were compared.Results:The alveolar lavage fluid CD3+and CD8+in CIP group were higher than those in non-CIP group,while the CD4+,CD4+/CD8+was lower than that in non-CIP group(t=5.387,7.674,7.415,7.845;P<0.001).The levels of IL-6,IL-8 and IL-10 in alveolar lavage fluid in CIP group were higher compared with those in non-CIP group(t=5.931,10.070,7.388;P<0.05).The levels of lymphocyte subsets and inflammatory mediators in alveolar lavage fluid of NSCLC patients had better efficiency on predicting CIP,and IL-6 had the highest predictive efficiency.When the cut-off value of IL-6 was>8.56 ng/L,the Youden index,area under the curve,sensitivity and specificity of predicting CIP were 0.546,0.852,96.87%and 57.69%respectively.Conclusion:Patients with NSCLC and CIP have excessive activation of lymphocyte subsets,and the inflammatory mediators are high,and the above related indicators can be used as biomarkers to predict NSCLC with CIP.
