Invasive-Noninvasive Sequential Mechanical Ventilation for the Treatment of Severe Pneumonia with Respiratory Failure

Abstract:Objective:To explore the clinical effects of invasive-noninvasive sequential mechanical ventilation in patients with severe pneumonia complicated by respiratory failure.Methods:A total of 98 patients with severe pneumonia and respiratory failure treated in Suyang Affiliated Hospital of Xuzhou Medical University from January 2020 to March 2024 were enrolled,and randomly divided into control group and observation group,with 49 patients in each group.The control group received invasive mechanical ventilation,while the observation group underwent invasive-noninvasive sequential mechanical ventilation.The outcomes measured included total duration of mechanical ventilation,duration of invasive ventilation,hospital stay,blood gas parameters[partial pressure of oxygen(PaO2),partial pressure of carbon dioxide(PaCO2),and pH value],incidence of ventilator-associated pneumonia,weaning success rate and quality of life scores[assessed using the SF-36 Health Survey(36-Item Short Form Survey,SF-36)]were compared.Results:The observation group showed significant reduction in total mechanical ventilation time,invasive ventilation duration,and hospital stay compared to the control group(t=4.455,5.080,3.511;P<0.05).Blood gas analysis revealed significantly higher PaO2 and pH values and significantly lower PaCO2 in the observation group compared to the control group(t=7.445,5.388,10.804;P<0.05).The incidence of ventilator-associated pneumonia was significantly lower and the weaning success rate was significantly higher in the observation group(x2=4.356,6.282;P<0.05).Quality of life scores were also significantly better in the observation group(t=7.853,P<0.05).Conclusion:Invasive-noninvasive sequential mechanical ventilation can effectively shorten the duration of treatment and hospital stay,improve blood gas parameters,reduce the incidence of ventilator-associated pneumonia,increase weaning success rates,and enhance the quality of life in patients with severe pneumonia complicated by respiratory failure.
