Impact of Different Drainage Pathways on Clinical Efficacy and Recurrence Rate in Patients Undergoing Functional Endoscopic Sinus Surgery

Abstract:Objective:To explore the impact of different drainage pathways on the clinical efficacy and recurrence rate in patients undergoing Functional Endoscopic Sinus Surgery(FESS).Methods:A total of 76 patients who underwent FESS from January 2020 to December 2022 in the hospital were selected for this study.Patients were divided into three groups based on the different drainage pathways opened during surgery:Group A(n=25),Group B(n=29),and Group C(n=22).Preoperative and postoperative comparisons were made in terms of endoscopic Lund-Kennedy scores,CT Lund-Mackay scores,pain levels[using Visual Analog Scales(VAS)],nasal ventilation function[including Nasal Minimum Cross-Sectional Area(NMCA),Nasal Cavity Volume(NCV),and nasal airway resistance],incidence of complications,and recurrence rates.Results:After treatment,the NCV of patients in group A was higher than that of the other two groups,and the nasal airway resistance was lower than that of the other two groups.The differences were statistically significant(F=5.419,7.011;P<0.05).The scores of the sphenoid sinus,anterior ethmoid sinus and sinusostomal passage complex in group C were significantly higher than those in the other two groups,and the differences were statistically significant(F=3.371,5.336,4.809;P<0.05).After the operation,the Lund-Kennedy score and VAS score of patients in group A were lower than those in the other two groups,and the differences were statistically significant(F=5.5531,3.861;P<0.05).The incidence of postoperative complications and the postoperative recurrence rate in group C were both higher than those in the other two groups,and the differences were statistically significant(χ2=7.170,6.038;P<0.05).Conclusion:Different drainage pathways significantly affect the clinical efficacy and recurrence rate in patients undergoing FESS.Each pathway,when successfully opened,is associated with a lower rate of recurrence and complications.Poor drainage through the frontal recess,middle meatus entrance,and sphenoid-ethmoid recess can increase the risk of postoperative recurrence and complications.
