Clinical Application Value of Monocyte Count in Severe Systemic Infections Caused by Ureteral Stones

Abstract:Objective:To evaluate the effectiveness of peripheral blood monocyte count as a diagnostic biomarker for predicting the risk of urosepsis and to analyze the potential correlation between monocyte count and urosepsis in patients with ureteral calculi.Methods:A retrospective study was conducted on 103 patients diagnosed with ureteral calculi admitted to Longmen County People's Hospital from December 2021 to February 2024.The patients were categorized into the urosepsis group(n=42)and the non-sepsis group(n=61)based on their clinical diagnosis.Peripheral blood monocyte count,white blood cell count,procalcitonin(PCT)levels,and urinary white blood cell count were compared between the two groups.The diagnostic value of these indicators in predicting the progression of ureteral calculi to urosepsis was assessed using receiver operating characteristic(ROC)curve analysis.Results:Patients in the urosepsis group exhibited significantly higher PCT levels and lower peripheral blood monocyte counts than those in the non-sepsis group,with statistically significant difference(t=9.595,7.322;P<0.001).ROC curve analysis demonstrated that compared with white blood cell count urinary leukocyte count,PCT and monocyte count had higher sensitivity and predictive accuracy for urosepsis.Furthermore,monocyte count was negatively correlated with PCT levels,with statistically significant difference(r=-0.48,P<0.001).Conclusion:A decrease in peripheral blood monocyte count plays a crucial role in predicting the progression of ureteral calculi to urosepsis and can serve as an important clinical monitoring biomarker.
