Thoracoscopic Subxiphoid Approach in the Treatment of Patients with Thymoma Combined with Myasthenia Gravis

Abstract:Objective:To investigate the comparative effectiveness of the thoracoscopic subxiphoid approach versus the right-sided approach in the treatment of patients with thymoma combined with myasthenia gravis(MG).Methods:Retrospective analysis of the clinical data of 160 patients with thymoma combined with MG who attended teh hospital and Xiangya Hospital from March 2022 to August 2023 were divided into two groups according to the different surgical approaches,128 cases in the control group for the implementation of the thoracoscopic right-sided approach and 32 cases in the observation group for the implementation of the thoracoscopic subxiphoid approach.Short-term efficacy,hospitalisation time,operation time,postoperative pain and complications and other perioperative related indicators of the two groups were compared.Results:There was no statistically significant difference in the clinical efficacy between the two groups of patients(Z=1.389,P=0.239).The operation time,indwelling time of the drainage tube and hospital stay in the observation group were significantly shorter than those in the control group,and the differences were statistically significant(t=2.842,3.447,4.768;P<0.05).One day after the operation,three days after the operation and one week after the operation,the VAS scores of both groups of patients decreased,and the observation group was significantly lower than the control group.The differences were statistically significant(t=5.166,7.254,10.523;P<0.05).Conclusion:Compared with the right-sided approach,the thoracoscopic raphe approach is more effective for patients with thymoma complicated by MG.The operation time is shorter,which can greatly reduce the drain retention time and hospital stay,and effectively reduce patients'postoperative pain.
