Clinical Comparative Study of Retrograde Indwelling Ureteral Catheter and Indwelling Double J Tube to Establish Artificial Hydronephrosis During Percutaneous Nephrolithotomy

Abstract:Objective:To explore the clinical effect of retrograde indwelling ureteral catheter and indwelling double J tube in the establishment of artificial hydronephrosis during percutaneous nephrolithotomy(PCNL).Methods:98 patients who underwent PCNL in the hospital from June 2018 to June 2023 were selected as the research subjects.According to the intraoperative indwelling tube methods,they were divided into observation group(n=49)and control group(n=49).The observation group was treated with indwelling double J tube,while the control group adopted indwelling ureteral catheter.The clinical conditions(channel establishment time,surgical time,stone clearance rate),blood indicators[serum creatinine(Cr),white blood cell(WBC),procalcitonin(PCT)]and renal function[urine white blood cell,urine microglobulin(MG),urine test positive rate]before surgery and 7 days after surgery were compared.The adverse reactions of the two groups at 1 week after surgery were recorded.Results:The success rate of catheterization in observation group was higher compared to control group(t=9.104,P<0.05).7 days after the operation,the PCT level in the control group was higher than that in the observation group,and the difference was statistically significant(t=2.455,P<0.05).7 days after the operation,the levels of white blood cells,MG and the positive rate of urine test in both groups decreased significantly,but there was no statistically significant difference between the two groups(t=0.923,0.618;χ2=0.343;P>0.05).The total incidence of adverse reactions in the observation group was lower than that in the control group,and the difference was statistically significant(χ2=9.245,P<0.05).Conclusion:The two indwelling regimens of ureteral catheter and double J tube during PCNL have significant effect on the establishment of artificial hydronephrosis during surgery,but indwelling double J tube is easier to operate and can reduce the risk of postoperative fever.
