Magnetic Resonance Multiple Sequence Imaging Quantitative Parameters for Breast Cancer Evaluation of Pathologi-cal Response After Neoadjuvant Chemotherapy

Abstract:Objective:To evaluate the value of magnetic resonance imaging(MRI)in the pathological response of breast cancer after NAC based on the changes of quantitative parameters before and after neoadjuvant chemotherapy(NAC).Methods:A total of 61 patients who were pathologically diagnosed with breast cancer and participated in NAC treatment in Sanmenxia Central Hospital from August 2022 to October 2023 were collected.MRI were used to evaluate the efficacy of NAC,and the patients were divided into significant response group(MHR)and non-significant response group(NMHR)with pathological results as the gold standard.The difference in the quantitative parameter values of the two groups of lesions was studied,ROC curve was drawn,and the logistic regression model was established to find out the parameters with high diagnostic efficacy,and to determine the therapeutic threshold,efficacy of the combined diagnosis of NAC.Results:61 breast cancer patients meeting the criteria were included,and divided into MHR group(n=21)and NMHR group(n=40)according to pathological results.After NAC,there were statistically significant differences in the maximum diameter change rate(△MD%),TIC curve type change rate(△TIC),early reinforcement rate change rate(△E%)and early reinforcement ratio change rate(△ER%)between the two groups(Z=-6.246,-3.291,-4.555,-4.624;P<0.05).After NAC,there was statistically significant difference in the distribution of TIC curve types between the two groups(Z=-4.109,P<0.001).ROC curve shows that the area under the curve(AUC)of △MD%,△E%,△ER%and △TIC were 0.985,0.827,0.885 and 0.733,respectively.Among them,the diagnostic efficiency of △MD%and △ER%was higher,and the AUC,sensitivity and specificity of combined diagnosis were 0.986,0.950 and 0.952,respectively.Hosmer-lemeshaw test P=0.978,the fit was good.Conclusion:Quantitative parameters of MRI imaging have certain value in predicting the curative effect of breast cancer before and after NAC,in which the maximum diameter change rate and early enhancement ratio change rate are more effective than other parameters,and the combined diagnosis is slightly better than single parameter.
