Analysis of Clinical Characteristics and Bacterial Drug Resistance in Patients with Klebsiella Pneumoniae and Escherichia Coli Bloodstream Infection

Abstract:Objective:To analyze the clinical characteristics of patients with Escherichia coli and Klebsiella pneumoniae bloodstream infection in The Second Affiliated Hospital of Harbin Medical University and The First People's Hospital of Jiande City,and to summarize the changing rules of bacterial drug resistance,so as to provide help for clinical diagnosis and treatment.Methods:The blood culture results of 742 patients with Escherichia coli and Klebsiella pneumoniae infection admitted to the hospital from January 2020 to August 2022 were collected retrospectively.The basic information,primary infection site,laboratory test,drug sensitivity test and prognosis were collected and analyzed retrospectively.Results:The mortality was higher in males,patients with pneumonia,hypoproteinemia,anemia,thrombocytopenia and bacterial CRE production.The resistance rate of Escherichia coli to sulfamethoxazole increased,and that of Klebsiella pneumoniae to cefoxitin,ertapenem,piperacillin/tazobactam,tegacycline and aztreonam increased.The resistance rates of Klebsiella pneumoniae and Escherichia coli to ceftazidime/avertam and tegacycline showed an upward trend.Conclusion:Most of the patients with Escherichia coli and Klebsiella pneumoniae bloodstream infection are 60~69 years old.Diabetes is the most common underlying disease in patients with Klebsiella pneumoniae and Escherichia coli bloodstream infection.Logistics regression analysis showed that pneumonia,bacterial production of CRE and hypoproteinemia are independent risk factors for death.
