Ultrasonic Characteristics and C-TIRADS Classification of Medullary Thyroid Carcinoma

Abstract:Objective:To analyze the ultrasonic characteristics of medullary thyroid carcinoma(MTC)and the classification of C-TIRADS(Chinese Ultrasonic Thyroid Image Reporting and Data System).Methods:69 nodule images confirmed by postoperative pathology as MTC from May 2018 to December 2022 in the First Affiliated Hospital of Zhejiang University School of Medicine were included,and 69 nodule images confirmed by postoperative pathology as papillary thyroid carcinoma(PTC)were extracted during the same period.The ultrasonic characteristics and C-TIRADS classification of nodule were analyzed.Results:There were significant differences between MTC and PTC nodules in margin,internal composition,A/T ratio,capsule invasion,blood flow and lymph node metastasis(χ2=4.328,14.725,34.922,9.215,24.193,9.763,21.130,5.432;P<0.05).There was significant difference in the overall classification of C-TIRADS between MTC group and PTC group(χ2=25.487,P<0.05).There was significant difference in 4a and 4c between MTC group and PTC group(χ2=12.522,18.681;P<0.05).Conclusion:In order to reduce the misdiagnosis rate,attention should be paid to lymph node scan in cases of low-echo cystic or solid nodules with smooth edges,A/T ratio≤1,high blood supply and punctate strong echo,and calcitonin should be determined for comprehensive evaluation.
