Clinical Application Value of Ultrasound Combined with Serum TNF-α and AFP Detection in the Diagnosis of Placental Implantation

Abstract：Objective:To investigate the clinical application value of ultrasound combined with serum tumor necrosis factor-α(TNF-α)and alpha-fetoprotein(AFP)detection in the diagnosis of placenta implantation.Methods:A total of 53 patients diagnosed with placenta accreta in the hospital from March 2018 to March 2024 were selected as the observation group,and a total of 106 healthy pregnant women who underwent prenatal care and normal delivery in the hospital during the same period were selected as the control group.The ultrasound results of the two groups,and the levels of AFP and TNF-α in the two groups were compared.The diagnostic efficacy of ultrasound combined with serum TNF-α and AFP detection was statistically analyzed.Results:The PSV and S/D levels in the observation group were significantly higher than those in the control group,and the RI and PI levels were significantly lower than those in the control group(t=4.751,9.870,9.336,7.447;P＜0.05).The levels of serum AFP and TNF-α in observation group were significantly higher than those in control group,with statistically significant difference(t=6.843,8.588;P＜0.05).PSV was positively correlated with serum AFP and TNF-α(r=0.563,0.628;P＜0.05).RI was negatively correlated with serum AFP and TNF-α(r=-0.616,-0.617;P＜0.05).PI was negatively correlated with serum AFP and TNF-α(r=-0.565,-0.632;P＜0.05).S/D was positively correlated with serum AFP and TNF-α(r=0.637,0.602;P＜0.05).Independent variables,such as uterine artery ultrasound parameters,serum AFP and TNF-α were included in logistic regression,and the analysis results were correlated with placenta implantation(OR=1.165,0.324,0.213,2.463,1.015,1.003;P＜0.05).ROC curve analysis showed that uterine artery ultrasound parameters,serum AFP and TNF-α had good diagnostic value for placenta implantation,and AUC values were 0.774,0.758,0.762,0.753,0.825 and 0.838,respectively.The AUC value of uterine artery ultrasonography combined with serum AFP and TNF-α was 0.896,which was significantly higher than that of each index alone.Conclusion:The detection of ultrasound combined with serum TNF-α and AFP can effectively improve the diagnostic value of placental implantation before delivery,so as to provide auxiliary theoretical evidence for prenatal intervention and clinical treatment.
