Application Value of DECT in Diagnosis of Bone Marrow Edema After Joint Injury

Abstract：Objective:To evaluate the diagnostic value of dual-energy CT(DECT)in traumatic joint bone marrow edema.Methods:Collecting the data of 37 patients with acute knee joint injuries who were treated at the Wuxi TCM Hospital from June to October 2003.All patients were examined by DECT to determine the presence of traumatic myeloedema.MRI images were used as the gold standard for quantitative analysis to evaluate the accuracy of virtual decalcification imaging in the diagnosis of traumatic knee bone marrow edema,and to compare the CT values of the bone marrow edema area and the adjacent normal bone marrow area.Receiver operating characteristic(ROC)curve was plotted to evaluate its diagnostic efficiency.Results:MRI showed 59 sites of bone marrow edema in 37 patients.The subjective judgment of 2 physicians on VNCa images confirmed a total of 60 and 57 bone marrow edema,with good consistency(Kappa 0.938).Based on dual-energy CT pseudo-color image,the sensitivity,specificity,positive predictive value,negative predictive value and accuracy were 96%,98%,97%,98%and 98%,respectively.The CT value of bone marrow edema area in VNCa was about(-16.08±27.97)HU,and that of healthy area was about(-86.58±15.41)HU,the difference was statistically significant(t=20.51,16.90;P＜0.01).Conclusion:DECT virtual decalcification imaging has shown high accuracy in the diagnosis of knee joint trauma-induced bone marrow edema,while the measurement of VNCa imaging CT values can effectively quantitatively analyze the situation of bone marrow edema.
