Analysis of the Incidence and Severity of Thrombocytopenia in Neonatal Sepsis with Different Pathogens

Abstract:Objective:To study the incidence,severity and duration of thrombocytopenia in neonatal sepsis with different pathogens,and to investigate the changes in platelet parameters in different pathogenic infections.Methods:44 children with neonatal sepsis combined with thrombocytopenia admitted to the hospital from January 2014 to December 2023 were selected for the study.Based on the blood culture results,the children were divided into 13 cases in the Gram-positive(G+)bacteria group,24 cases in the Gram-negative(G-)bacteria group and 7 cases in the fungi group.Platelet count(PLT)and platelet volume(MPV)were collected on day 1 of admission at the onset of sepsis and 48 h after the onset.The incidence and severity of thrombocytopenia,duration of thrombocytopenia,minimum platelet count and change in MPV were analyzed,and the difference in different pathogenic bacterial infections was compared.Results:The incidence of thrombocytopenia in neonatal sepsis was 12.2%,with the incidence of thrombocytopenia in G+bacterial sepsis,G-bacterial sepsis and fungal sepsis being 6.4%,17.8%and 33.3%,respectively.The incidence of thrombocytopenia was significantly lower in the G+bacterial group than in the G-bacterial and fungal groups.The rate of severe thrombocytopenia was significantly lower in the G+bacterial group than in the G-bacterial group.The minimum PLT in G-bacteria group and fungi group was significantly lower than that in G+bacteria group The duration of thrombocytopenia in G-bacteria group and fungi group was longer than that in G+bacteria group.The incidence of high MPV and maximum MPV in G-bacteria group were significantly higher than those in G+bacteria group,and the differences were statistically significant(χ2=19.114,7.185,6.799;P<0.05).Conclusion:Thrombocytopenia is common in neonatal sepsis,especially G-and fungal infections.Compared to G+organisms,neonatal sepsis caused by G-organisms and fungal infections has a higher incidence of thrombocytopenia,more severe platelet reductions,longer duration of thrombocytopenia,and lower minimum platelet counts.The MPV value of neonatal septicemia caused by G-bacteria infection is increased.Platelet parameters can be used to evaluate the severity of bloodstream infections and help in early differential diagnosis of bloodstream infection types.
