Diagnostic Efficacy of Serum Thyroid Hormone,HOMA-IR and HbA1c Levels in Gestational Diabetes Mellitus

Abstract：Objective:To explore the diagnostic efficacy of combined detection of serum thyroid hormone,insulin resistance index(HOMA-IR)and hemoglobin(HbA1c)levels in gestational diabetes mellitus.Methods:A retrospective analysis was conducted on 80 cases of gestational diabetes patients admitted to the hospital from May 2021 to May 2023,and 80 cases of healthy parturients who received health check-up in the hospital during the same period.Liver function tests combined with blood biochemical tests were performed on them.The levels of serum thyroid stimulating hormone(TSH),free tetraiodothyroxine(Ft4),free triiodothyroxine(Ft3),HOMA-IR and HbA1c of the patients were measured.Logistic multivariate regression model was established,the receiver operating curve(ROC)was further drawn,and the area under the curve(AUC)value was calculated.The diagnostic efficacy of combined detection of serum thyroid hormone,HOMA-IR and HbA1c levels in gestational diabetes mellitus were analyzed.Results:The levels of Ft4 in gestational diabetes mellitus group were lower than those in healthy group,and HOMA-IR and HbA1c were higher than those in healthy group,with statistically significant difference(t=6.661,6.214,9.203;P＜0.05).Multivariate Logistic regression model was constructed,and the results showed that Ft4,HOMA-IR,HbA1c were correlated with gestational diabetes mellitus(OR＞1,P＜0.05).The ROC curve was further drawn,and the results showed that the AUC of combined detection of Ft4,HOMA-IR and HbA1c in the diagnosis of hepatitis cirrhosis was all greater than 0.70,with certain predictive value,among which the combined detection had the highest value.Conclusion:Serum thyroid hormone,HOMA-IR and HbA1c levels are effective in the diagnosis of gestational diabetes mellitus.
