Clinical Efficacy and Prognostic Risk Factors of Postoperative"Sandwich Mode"Radiotherapy and Chemotherapy for Stage Ⅲ Endometrial Cancer

Abstract:Objective:To explore the clinical efficacy and prognostic risk factors of postoperative"sandwich mode"radiotherapy and chemotherapy in stage Ⅲ endometrial cancer patients.Methods:A total of 86 stage Ⅲ endometrial cancer patients from January 2017 to June 2018 who were admitted in Nanyang Central Hospital were selected.They were received"sandwich mode"radiotherapy and chemotherapy after surgery.The clinical efficacy was analyzed.Patients were followed-up for 5 years.The prognosis of the patients was statistically analyzed.Single factor and multiple factor logistic regression analysis were used to identify risk factors for prognosis.Results:Among the 86 patients with stage Ⅲ endometrial cancer,the total recurrence rate was 29.07%,the recurrence time was 11~42 months,and the average recurrence time was(29.34±2.60)months.The proportion of patients with recurrence-free survival time of 1 year,3 years and 5 years was 82 cases(95.35%),71 cases(82.56%)and 63 cases(73.26%),respectively.Compared with the good prognosis group,the poor prognosis group had a higher proportion of patients aged 60 years,pathological grade G3,non-endometrioid adenocarcinoma,depth of myometrium infiltration≥1/2,and lymphovascular space infiltration,with statistically significant difference(χ2=4.869,4.502,4.260,7.073,7.388;P<0.05).Multivariate logistic regression model showed that age≥60 years old,pathological grade G3,non-endometrioid adenocarcinoma,depth of muscle infiltration≥1/2,and lymphatic and vascular space infiltration were independent risk factors for prognosis of patients with stage Ⅲ endometrial cancer(OR=3.328,2.361,2.795,2.022,3.199;P<0.05).Conclusion:Postoperative"sandwich mode"radiotherapy and chemotherapy can achieve significant therapeutic effects in patients with stage Ⅲ endometrial cancer.However,the prognosis of patients is influenced by factors such as age,pathological grade,pathological type,depth of myometrial infiltration,and infiltration of lymphatic vessel spaces,which require special attention.
