Prognostic Value of Serum Anti-TgAb,Anti-TPOAb and TSH Levels in Patients with Thyroid Cancer After Total Thyroidectomy

Abstract：Objective:To investigate the predictive value of serum anti-thyroglobulin antibody(Anti-TgAb),anti-thyroid peroxidase antibody(Anti-TPOAb)and thyrotropin(TSH)combined detection in patients with thyroid cancer after total thyroidectomy.Methods:A retrospective study was conducted on 80 thyroid cancer patients who underwent total thyroidectomy in the hospital from October 2021 to October 2023.Based on the occurrence of recurrence and metastasis 1 year after surgery,the patients were divided into two groups:the occurrence group(n=28)and the non-occurrence group(n=52).The levels of serum Anti-TgAb,Anti-TPOAb,and TSH were compared between the two groups before and 2 months after surgery,and the differences were calculated.Clinical data were compared between the two groups.Logistic regression analysis was conducted to identify the factors affecting postoperative recurrence and metastasis.Receiver operating characteristic(ROC)analysis was used to evaluate the predictive value of the differences in preoperative and postoperative 2-month serum Anti-TgAb,Anti-TPOAb,and TSH levels for postoperative recurrence and metastasis.Results:The levels of serum Anti-TgAb,Anti-TPOAb and TSH in the post-operative group with the occurrence of group were higher than those in the non-occurrence group,and Anti-TgAb,Anti-TPOAb,TSH,Anti-TgAb△2,Anti-TPOAb△2 and TSH△2 were lower than those in the non-occurrence group(t=9.985,8.958,7.158;P＜0.001).Clinical stage,lymph node metastasis and tumor diameter were independent risk factors for postoperative recurrence and metastasis,while Anti-TgAb△2 Anti-TPOAb△2 and TSH△2 were independent protective factors for postoperative recurrence and metastasis(P＜0.05).The AUC of the combined prediction of Anti-TgAb△2,Anti-TPOAb△2 and TSH△2 for postoperative recurrence and metastasis was greater than that of individual indicators(P＜0.001).Conclusion:Serum Anti-TgAb,Anti-TPOAb and TSH levels can predict the recurrence and metastasis of patients with thyroid cancer after total thyroidectomy.
