Study on the Effect of Gallbladder Combined with Liver Tissue Wedge Resection Assisted Regional Lymph Node Dissection in the Treatment of Gallbladder Cancer

Abstract:Objective:To explore the clinical effect of laparoscopic cholecystectomy(LC)combined with wedge resection of liver tissue for the treatment of gallbladder cancer.Methods:102 patients with gallbladder cancer were randomly divided into LC group and combination group,with 51 cases in each group.LC group was treated with LC surgery.The combined group was treated with liver tissue wedge resection combined with LC+regional lymph node dissection,and both groups were followed up for 1 year.The short-term efficacy and prognosis of the two groups of patients were compared.Results:The CEA,CA199 and 125(CA125)levels in the combination group were lower than those in the LC group.The objective response rate(ORR)and disease control rate(DCR)in combination group were higher than those in LC group(t=2.759,2.576,2.397;P<0.05).The objective response rate(ORR)and disease control rate(DCR)in combination group were higher than those in LC group,with statistically significant difference(Z=2.521,2.606;P<0.05).At 24 h,48 h and 72 h after surgery,the visual analogue scale(VAS)score of the combined group was higher than that of the LC group,with no statistically significant difference(t=1.868,1.839,0.895;P>0.05).The complication rate of the combination group was 9.80%higher than that of the LC group(7.84%),and the difference was not statistically significant(χ2=0.102,P>0.05).Median progression-free survival(PFS)and median overall survival(OS)in the combination group were higher than those in the LC group,and the recurrence rate was lower than those in the LC group,with statistically significant difference(t=3.351,2.770;χ2=7.141;P<0.05).Conclusion:LC combined with liver tissue wedge resection+regional lymph node dissection can improve the short-term efficacy of patients with gallbladder cancer.It can prolong the patient's survival cycle and reduce the risk of disease recurrence without affecting postoperative pain or increasing the risk of complications.
