Analysis of High-risk Factors for Postoperative Dry Eye in Patients with Cataract and Angle Closure Glaucoma

Abstract:Objective:To explore the high-risk factors of dry eye syndrome in patients with cataract and angle closure glaucoma(PACG)after surgical treatment.Methods:The clinical data of 92 cataract patients with PACG admitted to the hospital from January 2021 to December 2022 were retrospectively analyzed.All patients were operated on.After surgery,they were divided into two groups according to whether there was dry eye within 3 months.The age,gender,lesion site,course of disease,body mass index,incision length,diabetes,hypertension,total ultrasound energy release during surgery,exposure time under surgical microscope,eyelid fat viscosity,conjunctival laxity grading,meibomian gland dysfunction,vitreous hemorrhage,and eye hygiene were analyzed with univariate analysis and logistic regression analysis to obtain independent risk factors.Results:A total of 33 out of 92 patients developed dry eye syndrome after surgery,with an incidence rate of 35.87%(33/92).There were significant differences between the two groups in diabetes,exposure time under operating microscope,eyelid fat viscosity,conjunctival relaxation grade,and meibomian gland dysfunction(x2=32.210,21.874,7.908,15.721,11.246;P<0.05).Patients with diabetes mellitus(OR=17.086,95%CI=5.782~50.485),intraocular microscope exposure time≥20 min(OR=9.008,95%CI=3.394~23.912),eyelid fat viscosity ≥ grade 3(OR=3.504,95%CI=1.437~8.546),conjunctival relaxation grade ≥2(OR=6.192,95%CI=2.421~15.839),meibomian gland dysfunction(OR=12.391,95%CI=2.518~60.978)was a high risk factor for dry eye in cataract patients with PACG after surgery(P<0.05,OR>1).Conclusion:The high risk factors for dry eye in patients with cataract combined with PACG include diabetes mellitus,exposure time≥20 min under microscope,palpebral fat viscosity≥3,conjunctival laxity grade≥2,and meibomian gland dysfunction.It is necessary to identify high-risk groups early and take preventive measures to reduce the incidence of dry eye in patients with PACG.
