Abstract:Objective:To compare the application effects of different nasal feeding regimensin the patients receiving mechanical ventilation by extracorporeal membrane oxygenation. Methods:A total of 120 patients enrolled from ICU of the First People's Hospital of Jiande City from January 2020 to January 2023 were assigned to 3 groups in accordance with the random number table:group A(n=40),group B(n=40)and group C(n=40).Group A was treated with nasal feeding for 6 h at an interval of 2 h,group B was treated with nasal feeding for 5 h at an interval of 2 h,and group C was treated with nasal feeding for 24 h.The target caloric attainment and nutritional indexes, the incidence of ventilator-associated pneumonia(VAP),the duration of mechanical ventilation and hospital stay,as well as the feeding intolerance of diarrhea, constipation, gastric retention and pH of gastric fluid in the 3 groups were compared. Results:The EN standard number of patients in the 3 groups was higher in group B than in group A and group C.The score of NRS-2002 in group B was lower than that in group A and group C,and the difference was statistically significant(t/x2=10.155,10.155;P=0.006).The VAP infection in group B was less than that in group A and group C.The duration of mechanical ventilation and ICU stay in group B were significantly different from those in group A and group C(t/x2=-l.000,2 026.055,920.142;P<0.05).The gastric retention and pH value of gastric fluid in group B were lower than those in group A and group C,and the difference was statistically significant(f/x2=9.088,747.539;P<0.05).Conclusion:Continuous nasal feeding for 24 h may not be conducive to the gastrointestinal rehabilitation of severe patients. If adjusted to continuous treatment for 5 h,2 h interval nasal feeding treatment may be more conducive to the rehabilitation of patients.
