Effect of Chaigan Jieyou Decoction in the Treatment of Coronary Heart Disease with Depression and Anxiety,and its Influence on Inflammatory Factors and Neurotransmitters

Abstract：Objective:To observe the effect of Chaigan Jieyou Decoction in treating coronary heart disease(CHD)with depression and anxiety,and its influence on inflammatory factors and neurotransmitters.Methods:A total of 78 patients with CHD complicated with depression and anxiety who were admitted to the Third Affiliated Hospital of Gannan Medical University from January to July 2023 were selected,and they were randomly divided into the control group and the experimental group,with 39 cases in each group.The control group was given conventional western medicine treatment,while the experimental group was treated with Chaigan Jieyou Decoction on the basis of conventional western medicine treatment.The two groups were compared in terms of curative effect,cardiac function indicators[serum cardiac troponin I(cTnI)and N-terminal prohormone of brain natriuretic peptide(NT-proBNP)],inflammatory factors[serum interleukin-6,IL-1β and tumor necrosis factor(TNF)-α],neurotransmitters[serum norepinephrine(NE)and 5-hydroxytryptamine(5-HT)],heart rate variability(HRV)indicators[time domain(SDANN),root mean square of successive differences(RMSSD)and percentage of adjacent NN intervals differing by more than 50 milliseconds(PNN50)],and frequency domain indicators[high-frequency band(HF),low-frequency band(LF)and very low-frequency band(VLF)].Results:There was statistically significant difference in the total effective rates between the two groups(x2=9.716,P＜0.05).After treatment,the levels of myocardial injury markers cTnl and NT-proBNP in the experimental group were lower than those in the control group,and the difference was statistically significant(t=4.309,3.909;P＜0.05).After treatment,the levels of inflammatory cytokines(TNF-α,IL-6,IL-1 β)and neurotransmitters(NE,5-HT)in the experimental group were better than those in the control group,with statistically significant difference(t=13.085,8.610,9.347,7.270,8.280;P＜0.05).After treatment,the time domain indexes SDNN,SDANN,RMMSD and PNN50(％)were increased in both groups,and the differences were statistically significant(t=4.126,3.408,8.156,4.477;P＜0.05).After treatment,the improvement of HRV parameters in time and frequency domains in the experimental group was significantly better than that in the control group,and the difference was statistically significant(t=2.057,4.846,3.489;P＜0.05).After treatment,the LEVF of the experimental group was higher than that of the control group,and the difference was statistically significant(x2=3.836,6.744;P＜0.05).Conclusion:Chaigan Jieyou Decoction is effective in treating patients with CHD complicated with anxiety and depression.It can improve the patients'myocardial enzymes,inflammatory response and neurotransmitter levels;and regulate HRV.
