Analysis of the Risk Factors of Delayed Bleeding After Early Gastric Cancer ESD Surgery

Abstract:Objective:To observe the effectiveness of hemostatic clip assisted endoscopic mucosal dissection(ESD)in the treatment of early gastric cancer,and analyze the risk factors for delayed bleeding.Methods:122 early gastric cancer patients from January 2022 to March 2023 were retrospectively analyzed.The incidence of delayed bleeding after surgery was statistically analyzed.The influencing factors of delayed bleeding after hemostatic clip assisted ESD surgery were summarized.Results:Results of multivariate logistic regression analysis showed that,tumor diameter>20 mm,tumor located in the lower part of the stomach,tumor infiltration depth≥500 μm,preoperative biopsy times≥2,and postoperative disuse of antiplatelet drugs<3 days were all risk factors affecting the effect of hemostatic clip-assisted ESD treatment(OR=5.736,5.281,5.339,5.772,8.012;P<0.05).Conclusion:Hemostatic clips can to some extent prevent and treat delayed postoperative bleeding in early gastric cancer after ESD surgery.However,for patients with larger and more deeply infiltrating lower gastric tumors,preoperative biopsy frequency should be reduced as appropriate,and antiplatelet drugs should be discontinued as early as possible.
