Correlation Between Renal Function Indicators and Umbilical Artery Blood Flow Parameters in HDP and Prediction of Adverse Prognosis

Abstract:Objective:To analyze the correlation between renal function indicators and umbilical artery blood flow parameters in pregnancy induced hypertension(HDP),and the predictive efficacy of adverse prognosis in patients.Methods:58 HDP patients were selected as the case group,and 60 pregnant women who underwent routine physical examinations were selected as the control group.Renal function indicators and umbilical artery hemodynamic parameters were compared,and the correlation between renal function indicators and umbilical artery hemodynamic parameters were analyzed.HDP patients were divided into poor prognosis group and good prognosis group,and the predictive efficacy of renal function indicators on poor prognosis of HDP were analyzed.Results:The ratio of uric acid(UA),24 h urinary protein(24 h PRO),arterial pulsation index(PI),resistance index(RI),systolic peak and end-diastolic flow velocity(S/D)in case group was higher than that in control group,and the differences were statistically significant(t=5.693,5.692,31.537,12.508,19.012;P<0.05).UA,24 h PRO and other renal function indexes were positively correlated with PI,SI,S/D of umbilical artery(r=0.252,0.254,0.251;r=0.253,0.255,0.252).UA,24 PRO,PI,SI and S/D in the poor prognosis group were higher than those in the good prognosis group,with statistically significant difference(t=2.432,2.545,2.863,2.651,3.659;P<0.05).Logistic regression analysis showed that UA,24 h PRO,PI,RI and S/D were the risk factors leading to adverse prognosis of HDP(OR=5.772,6.033,4.415,6.331,4.339).The higher the level of UA and 24 PRO,the higher the risk of poor prognosis of HDP.Conclusion:UA and 24 h PRO will increase with the increase of umbilical artery hemodynamic indicators:PI,SI and S/D.It has high predictive power for poor prognosis of HDP.
