Relationship Between Serum ENA-78,HIF-1α and the Severity of Endometriosis,and the Prediction of Moderate and Severe Pelvic Adhesion

Abstract：Objective:To analyze the relationship between epithelial neutrophil activating peptide-78(ENA-78),hypoxia inducible factor 1α(HIF-1α)and the severity of endometriosis,and the prediction of moderate and severe pelvic adhesion.Methods:124 patients with endometriosis who underwent surgical treatment in the hospital from February 2019 to February 2023 were included in the observation group,and physical examination work in the same period of 124 cases of healthy women in the control group.The expression levels of ENA-78,HIF-1α and cancer antigen 125(CA125)in serum of the two groups were detected,and the endometriosis staging(r-AFS)score of the American Society for Reproductive Medicine was performed to analyze the relationship between the expression levels of ENA-78 and HIF-1α in serum and the score of CA125 and r-AFS.Using the area under the receiver-operating characteristic(ROC)curve(AUC)evaluation of serum alpha ENA-78,HIF-1 prediction of moderately severe pelvic adhesions.Results:Serum expression levels of ENA-78 and HIF-1α in observation group were higher than those in control group,and the differences were statistically significant(t=27.414,32.452;P＜0.001).The expression levels of ENA-78 and HIF-1α in serum in groups Ⅲ to Ⅳ were higher than those in groups Ⅰ to Ⅱ,and the differences were statistically significant(t=20.401,20.883;P＜0.001).The expression levels of ENA-78 and HIF-1α in serum of endometriosis patients were positively correlated with CA125 and r-AFS scores,and the differences were statistically significant(P＜0.05).According to ROC curve analysis,the optimal cutoff values of serum ENA-78 and HIF-1α for predicting the occurrence of moderate and severe pelvic adhesion in patients with endometriosis were 5.46 ng/mL and 15.78 pg/L,and the AUC was 0.632 and 0.658,respectively,and the combined prediction AUC was 0.918,which was greater than the AUC of single index.The difference was statistically significant(P＜0.05).Conclusion:Serum ENA-78 and HIF-1α are related to the progression of endometriosis,which can help to reflect the severity of endometriosis and predict the combined effect of moderate and severe pelvic adhesion.
