Significance of Alveolar Lavage Fluid sTREM-1 on Lower Respiratory Tract Infection with Klebsiella Pneumoniae

Abstract:Objective:Klebsiella pneumoniae infection and colonization are difficult to distinguish.To observe the difference of sTREM-1,to provide a high sensitivity and strong specificity test index for clinics.Methods:45 patients admitted to Pulmonary Department of the hospital from March 2021 to June 2023 were selected as the study objects.Among them,15 patients with the growth of Klebsiella pneumoniae in sputum culture were judged to be infected into the infection group,15 patients with the growth of Klebsiella pneumoniae in sputum culture were judged to be colonized into the colonization group,and 15 patients with no pathogenic bacteria culture and no pulmonary infection were included in the control group.The difference of sTREM-1 concentration in bronchoalveolar lavage fluid(BALF)among the three groups,and the significance of sTREM-1 in the diagnosis of pulmonary infection was analyzed.Results:There were no significant differences in gender and age among the three groups(F=1.228,χ2=2.143,P>0.05),but there were significant differences in quality of life scores and CPIS scores among the three groups(K=21.274,37.761;P<0.05).There were significant differences in the levels of procalcitonin(PCT),C-reactive protein(CRP)and sTREM-1 among three groups(K=20.029,14.856,31.62,P<0.05).The specificity of serum PCT in the diagnosis of infection was high,but the sensitivity was low.CRP was highly sensitive to the diagnosis of infection,but was less specific.Compared with the above two,the level of sTREM-1 in bronchoalveolar lavage fluid had higher sensitivity and specificity for the diagnosis of infection.Conclusion:The data show that sTREM-1 is significantly better than CRP,PCT and other inflammatory indicators in the diagnosis of Klebsiella pneumoniae infection and colonization.
