Predictive Analysis of Carotid Artery Plaque Neovascularization Evaluated by Contrast-enhanced Ultrasound for Recurrence in Patients with Acute Cerebral Infarction

Abstract:Objective:To explore the predictive value of quantitative analysis of carotid artery plaque neovascularization by contrast-enhanced ultrasound in the recurrence of acute cerebral infarction patients.Methods:235 patients with acute cerebral infarction accompanied by carotid artery plaque admitted to Chinese Medicine Hospital of Puyang from June 2020 to June 2022 were included as the research object.The carotid artery plaque of patients was examined by contrast-enhanced ultrasound,and the plaque neovascularization parameters were obtained.The patients were followed up for 12 months after intravenous thrombolytic therapy,and the incidence of acute cerebral infarction during the follow-up period was statistically analyzed.The contrast-enhanced ultrasound parameters of carotid artery plaque neovascularization were compared between the two groups,and the receiver operating characteristic(ROC)curve was drawn to analyze the value of quantitative analysis of carotid artery plaque neovascularization in the prediction of recurrence in patients with acute cerebral infarction.Results:During the follow-up,231 of the 235 patients completed the follow-up work.During the follow-up of 231 patients,acute cerebral infarction recurred in 51 cases,accounting for 22.08% .The score of NHISS in recurrence group was higher than that in non recurrence group at admission,and the number of patients with hypertension and atrial fibrillation was higher than that in non recurrence group(χ2=3.545,P<0.05;t=0.480,22.135;P<0.05).The plaque EI and Ratio values of the recurrence group were higher than those of the non recurrence group(t=8.225,10.391;P<0.05).The results of drawing ROC curves showed that the area under the curve(AUC)of plaque EI and Ratio values for predicting recurrence in patients with acute cerebral infarction after intravenous thrombolysis treatment was≥0.7,both of which had certain predictive value.Conclusion:Contrast-enhanced ultrasound can evaluate the abundance of neovascularization in carotid artery plaques in patients with acute cerebral infarction.High plaque EI and Ratio values are associated with disease recurrence and can serve as early warning indicators for acute cerebral infarction recurrence.
