Application Effect of Single-aperture Thoracoscopic Lobectomy in Young Patients with Pulmonary Tuberculosis

Abstract:Objective:To explore the efficacy of single-orifice thoracoscopic(SITS)lobectomy in the treatment of young patients with pulmonary tuberculosis(PTB).Methods:The data of 94 young patients with PTB in the hospital were retrospectively collected,and divided into 2 groups according to different surgical protocols.47 patients treated with SITS lobectomy were classified as group A,and 47 patients treated with three-hole thoracoscopic lobectomy(TTTL)were classified as group B.The operative indexes(drainage tube indwelling time,intraoperative blood loss,postoperative 24 h induced flow,length of operation and hospital stay],postoperative rest and cough pain numerical scale(NRS)score,pulmonary function before and after surgery lung function[forced expiratory volume in the first second(FEV1),maximum expiratory flow rate(PEF),maximum vital capacity(FVC)],and complications were compared between the two groups.Results:The surgical duration of group A was longer than that of group B,and the hospitalization duration was shorter than that of group B,with statistically significant differences(t=9.501,5.271;P<0.05).The NRS scores at 2 hours,24 hours,and 48 hours after surgery in group A were lower than those in group B,with statistically significant differences(t=3.028,7.659,4.824,2.582,8.031,13.051;P<0.05).3 months after surgery,the levels of FEV1,PEF and PEF of two groups were significantly increased compared to before surgery,with statistically significant differences(t=7.994,7.354,6.103,5.270,11.658,12.236;P<0.05).The incidence of complications in group A was 2.13%lower than 17.02%in group B,and the difference was statistically significant(χ2=6.021,P<0.05).Conclusion:TTTL and SITS lobectomy are both effective for the treatment of young patients with PTB,and they are equally effective in improving lung function.However,the latter can further reduce the pain degree of patients with postoperative rest and cough,reduce the risk of complications,and shorten the rehabilitation process,but the operation is difficult.It may slightly lengthen the time of surgery.
