Risk Factors of Complications After Total Hip Joint Replacement and Correlation Analysis with Hip Biomechanics

Abstract:Objective:To analyze the risk factors for postoperative complications of THA and their correlation with hip biomechanics.Methods:This was a retrospective study.The case included 121 patients treated with THA in Pingdingshan First People's Hospital from March 2020 to September 2022.All patients were followed up for half a year after surgery.According to the follow-up,30 patients with postoperative complications were listed as Group A,and 91 patients without postoperative complications were listed as Group B.The risk factors that can lead to postoperative complications of THA were summarized,the correlation between the risk of postoperative complications and the biomechanics of the hip joint was analyzed.Results:Logistic regression analysis showed that age≥65 years old,female,with diabetes,postoperative total hip height>50 mm,femoral height<60 mm,acetabular height difference<3 mm,total hip offset<70 mm,femoral offset<35 mm,acetabular offset<30 mm,and cup diameter>50 mm were the risk factors for postoperative complications of THA(OR=5.325,5.325,5.229,5.281,5.334,5.125,5.616,6.025,6.331,5.959;P<0.05).According to Spearman correlation coefficient test,the total hip joint height,femoral height,difference in acetabular height,femoral offset,and acetabular offset were negatively correlated with the incidence of complications after THA surgery(r=-0.154,-0.144,-0.161,-0.156,-0.147,-0.155),while cup diameter was positively correlated with the incidence of complications(r=0.252).Conclusion:The biomechanical abnormality of hip joint will affect the efficacy of THA in elderly female patients with diabetes and long-term history of hormone use.The smaller the total hip joint height,femoral height,acetabular height difference,femoral offset,and acetabular offset,etc.The larger the cup diameter after surgery are,the higher the risk of complications is.
