Study on the Relationship between Changes in the Levels of Amino-terminal Brain Natriuretic Peptide Precursor,Calcitoninogen and Interferon Regulatory Factor 4 and the Condition and Prognosis of Children with Sepsis

Abstract:Objective:To study the relationship between changes in the levels of amino-terminal brain natriuretic peptide pre-cursor(NT-proBNP),procalcitonin(PCT),and interferon regulatory factor 4(IRF4)and the condition and prognosis of children with sepsis.Methods:108 cases of children with sepsis admitted to the hospital from February 2018 to June 2020 were selected as the study group,and another 58 cases of children with health checkups during the same period were selected as the control group.The levels of NT-proBNP,PCT and IRF4 were compared in different populations and with different prognoses.ROC curves were plot-ted to analyze the predictive value of the levels of the above factors for pediatric sepsis death.Results:The children in the study group had significantly higher levels of NT-proBNP and PCT and lower levels of IRF4 than those in the control group,with statisti-cally significant differences(t=27.073,4.707,120.612,P<0.05).As the condition of children with sepsis progressed,the levels of NT-proBNP and PCT gradually increased,and the level of IRF4 gradually decreased,and the difference was statistically signifi-cant(F=58.84,21.92,15782.80,P<0.05).Of the 108 children with sepsis,86(79.63%)had a favorable prognosis,22(20.37%)had a poor prognosis,and 16 died.The levels of NT-proBNP and PCT in the good prognosis group were significantly lower than those in the poor prognosis group,and the levels of IRF4 were significantly higher than those in the poor prognosis group,and the differences were statistically significant(t=10.414,10.401,125.323,P<0.05).The results of the ROC curve analysis showed that the levels of NT-proBNP,PCT and IRF4 had a high predictive value for death in children with sepsis,with the area under the curve of the combined test being the largest at 0.899,and the area under the curve of the single test being the largest at 0.804 for NT-proBNP.Conclusion:The high expression of NT-proBNP and PCT and the low expression of IRF4 in children with sepsis may be closely related to the prognosis and condition of children with sepsis,and the detection of the levels of the above factors can help to assess the prognosis of the children.
