Development of Exercise Rehabilitation Review Indicators and its Obstacle Factors in Older Heart Failure Patients

Abstract:Objective To implement evidence-based exercise therapy in older heart failure patients,develop review indicators,analyze the barriers and enablers factors,and develop reform tactics.Methods Clinical concerns were identified,literature was retrieved,the evidence was evaluated and summarized,and review indicators were created based on the Joanna Briggs Institute's(JBI)theoretical model of evidence-based health care in Australia.From May to July 2024,a baseline review was carried out to identify obstacles and facilitators and develop plans for transformation.Results The study comprised 24 pieces of evidence in total,and 28 review indicators were created.Only four of them had a review implementation rate of 100%,twelve had a rate of 0%,and the remaining indicators ranged from 40%to 80%.An examination of the integrated-Promoting Action on Research Implementation in Health Services(i-PARIHS)framework identified 26 barriers,such as medical staff members'ignorance of exercise rehabilitation,and 26 enablers,such as the directors of the nursing and cardiovascular internal medicine departments'strong support for this evidence-based transformation initiative.Twenty-one transformation methods were also created,including systems and procedures for exercise rehabilitation.Conclusion The study produces review indicators that can be measured objectively.The best evidence for exercise rehabilitation in elderly heart failure patients differed significantly from real practice.To support the clinical transformation of evidence,clinical improvement should be implemented for both promoting and barrier aspects.
