The Mediating Effect of Insomnia Severity between Kinesiophobia and Pain Catastrophizing in Patients with Ankle Injuries

Abstract:Objective To explore the mediating effect of insomnia severity between kinesiophobia and pain catastrophizing in patients with ankle injuries.Methods A total of 482 patients diagnosed with ankle injuries were investigated using the General Information Questionnaire,Tampa Scale for Kinesiophobia(TSK-11),Insomnia Severity Index(ISI),and Pain Catastrophizing Scale(PCS).Results The median scores(interquartile range,IQR)for kinesiophobia,insomnia severity and pain catastrophising were 13.00(11.00,19.00),10.00(7.00,14.00)and 14.00(13.00,18.00)respectively,and there were pairwise positive correlations among the three variables(P<0.01).Insomnia severity played a partial mediating role between kinesiophobia and pain catastrophizing,accounting for 14.93%of the total effect.Conclusion Ankle injury patients do not exhibite pain catastrophizing.Insomnia severity is a mediating variable between kinesiophobia and pain catastrophizing in patients with ankle injuries.Healthcare professionals should take sleep management as a core intervention component,interrupt the pain catastrophizing pathway triggered by kinesiophobia by addressing patients'insomnia issues,help them adopt health-promoting lifestyles,and thereby reduce patients'kinesiophobia and pain levels.
